I hereby grant Frontier and its affiliates my permission to the rights of my image, likeness and sound of my voice recorded on photograph, audio or video for Frontier’s testimonial / referral program (the “Program”). I understand that my recorded image, sound of my voice, quotes, comments and / or referrals, may be edited, copied, exhibited, published and distributed for internal and external business use and waive the right to inspect or approve the finished product. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image, recording, quotes, or comments except pursuant to the Program. I understand that the term “photograph” as used herein encompasses both still photographs and motion picture footage. I hereby release Frontier Communications, and any of its associated or affiliated companies, their directors, officers, agents, employees and customers, and appointed advertising agencies, their directors, officers, agents and employees from all claims of every kind on account of such use.

On behalf of the Company, I have the authority to grant and I hereby grant Frontier and its affiliates permission to use the Company name and logo in connection with Frontier’s testimonial / referral program without payment or any other consideration. The Company agrees and understands that the Company name and logo copied, exhibited, published and distributed for internal and external business use in connection with the Program. The Company waives any right to royalties or other compensation arising or related to the use of the name or logo except per the Program. The Company hereby releases Frontier Communications, and any of its associated or affiliated companies, their directors, officers, agents, employees and customers, and appointed advertising agencies, their directors, officers, agents and employees from all claims of every kind on account of such use.
